Cognitive Dementia and Memory Service

The Cognitive Dementia and Memory service (CDAMS) is a multi-disciplinary, diagnostic clinic for older people experiencing change to memory or other areas of thinking.

In addition to diagnosis the clinic provides information, education, support and referral to other community agencies for all affected by cognitive change.

Eligibility Criteria

  • Older people with cognitive decline – not previously diagnosed. The clinic is able to provide a second opinion in regard to diagnosis.  
  • Must live in an eligible postcode listed below.

This service is not for:

  • for the ongoing management of dementia​
  • to start on cholinesterase inhibitors​
  • for a stand-alone neuropsychological assessment​
  • for competency/capacity assessments​
  • unstable mental health conditions requiring active psychiatric input​.
  • Client with known diagnosis unless a specialist second opinion is required.

Additional information required

NOTE: If a referral is made by an inpatient unit a letter will be sent to the GP requesting the ‘Required Baseline Investigations’. Please advise the GP in the Discharge Summary and client that a CDAMS referral has been made and that further information will be requested.

Referrals should include:

  • Medical Letter
  • Discharge Summary
  • All other patient demographic and clinical details and:

REQUIRED BASELINE INVESTIGATIONS:

Pathology & Radiology (bloods within the last 3 months)
  • CT brain or MRI brain within the past 12 months
  • ECG
  • FBE
  • TSH
  • ESR
  • Serum Vit B12
  • Serum urea, electrolytes, creatinine
  • Serum Calcium
  • Random blood glucose
  • Vitamin D
  • LFTs
  • Syphilis serology **only required if indicated**
Complete Cognitive Screening Tests:
  • Copy of MMSE (or similar test) score and date tested. If the MMSE result is 19/30 or below, provide client’s support person with IQCODE to complete and forward.

Please assess and manage any conditions that may impact cognition especially:

Assessment for Depression/Anxiety
  • Geriatric Depression Scale (GDS): · Complete GDS which has been sourced from the Dementia Outcomes Measurement Suite (DOMS). If GDS score is 5 or above, commence treatment.
Sleep apnoea (OSA).  If required, refer for sleep study through Monash Lung and Sleep.
