Post acute care

This program seeks to ensure a safe discharge from hospital, including the emergency department, by providing people with community based supports to help them recover in their home or community and to reduce the risk of readmission to hospital.

Post Acute Care (PAC) provides short-term community-based services/home-based treatment and support to help patients recover at home after leaving the hospital. It aims to assist people, especially those who live alone, have limited support, or are usually carer. Where another program was providing support prior to the hospital episode, it is expected that these services will resume on discharge from the hospital. PAC provides a brokered service model and, if unable to source suitable services, will feed back to the referrer and patient.

Services include:
  • Community nursing. Note: Please ensure clients are provided with equipment i.e. 3 days worth of dressing supplies, IDC, Nephrostomy care, BGL equipment, as community nurses do not carry them between visits.
  • Personal care
  • Home care
  • Physiotherapy

Eligibility Criteria

This service is for:

  • People aged 18 years and over
  • People discharged from a public hospital or public emergency department in the last 28 days
  • Have a new support need related to the hospital or emergency department admission

This service is not for:

  • Patients whose home environment is not safe for visiting
  • Patients where another service was providing support prior to admission. Note: it would be expected these services continue to support at home (e.g. NDIS, Home Care Packages 3 and 4, TAC and Workcover)
