The OPCH is a home-based outreach model supporting older people at higher risk of avoidable hospital presentations. By providing early, focused support, the program helps older people stay well at home—aligning with our commitment to the right care, at the right time, in the right place.

How it will work

From 7 April 2026, Monash Health will pilot the service in partnership with the Victorian Virtual Emergency Department (VVED).

Eligible older people presenting to the VVED may be referred to the Older Persons Care Hub (OPCH) for short‑term specialist geriatric assessment and multidisciplinary care delivered in their home. Activated when specialist intervention is needed but inpatient care is not, this model provides timely, targeted support in a familiar environment while reducing non‑urgent Emergency Department presentations. It is expected that the Older Persons Care Hub will lead to better outcomes, fewer hospital visits, and stronger support pathways for older people to be safely managed in the community.

Patients will be discharged with appropriate referrals, and their GP will remain responsible for ongoing care.

Referral Pathway

  • Referred by the Victorian Virtual Emergency Department (VVED)

Referral Criteria

The patient:

  • is >65yo or >50yo Aboriginal or Torres Strait Islander
  • lives at home
  • lives in the City of Casey or Cardinia Shire
  • has non-emergency acute needs

Exclusion Criteria

The patient:

  • has emergency needs
  • is not safe to remain at home until the next working day

Important information

This is a PILOT program. At this stage it can only be accessed via a referral from the VVED.

Urgent care is when you need medical attention for an illness or injury that can be managed without a trip to the emergency department but cannot wait for a regular appointment with a general practitioner (GP).

  • minor infections
  • minor fractures, sprains, sports injuries and neck and back pain
  • urinary tract infections (UTIs)
  • mild burns
  • minor cuts
  • respiratory illness
  • gastroenteritis
  • insect bites and rashes
  • sexually transmitted infections (STIs)
  • minor eye and ear problems

Victorian Virtual Emergency Department

The Victorian Virtual Emergency Department (VVED) allows you to speak with emergency doctors and nurse practitioners 24 hours a day, 7 days a week.

You must register online to contact VVED; it is not available via a standard phone call. As your consultation will be via video, you will need a device with a camera to use the VVED. 

If you are eligible, you will be connected from your home, workplace, aged care facility or GP clinic to doctors and nurse practitioners who are trained to assist you. Interpreter services are available.

The VVED is a free service.
