Family Violence Information Sharing Scheme and Child Information Sharing Scheme

From 19 April 2021, all Monash Health program areas became prescribed Information Sharing Entities (ISE) under the Family Violence Protection Act (2008) and Child Wellbeing and Safety Act (2005).

Monash Health has developed a process for Information Sharing Entities (ISEs) and Risk Assessment Entities (RAEs) to request relevant information under the Family Violence Information Sharing Scheme (FVISS) and the Child Information Sharing Scheme (CISS). The FVISS and CISS complement existing release of information processes, as outlined below.

How to request information under FVISS and CISS if you are a prescribed agency

To request information from Monash Health under either scheme:

Complete the Monash Health Information Sharing Request form.
Ensure that thresholds for sharing have been met, including consent requirements. Refer to the Victorian Government website if you require further details.
Email the completed form to ISS@monashhealth.org.
Monash Health will review the request and provide a response.
