Release of Information – Accessing Patient Information

The Release of Information (ROI) Unit is a centralised unit operating from Monash Medical Centre. The ROI Unit processes requests for patient information for all Monash Health sites.

All requests should be forwarded to the ROI Unit via email.

The ROI Unit email is not monitored outside of business hours. For urgent requests, please call (03) 9594 2112.

  • Urgent out of hours requests will be processed by the Scanning Centre (Clayton) or Nurse Coordinators at each site.
  • The ROI Unit email is not monitored outside of business hours. For urgent requests, please call (03) 9594 2112.
  • All non-urgent requests received out of hours will be processed by the ROI Unit the following business day.

Child Protection

A formal request under Section 192 of the CYFA 2005, in addition to:

  • Name and DOB of the person you are requesting information about
  • The specific information required, e.g. date range or episode of care – we will not provide routinely the entire medical record
  • The above can be outlined in the body of an email or the formal request on your letterhead
