Kingston Satellite Dialysis and Transitional Care Unit named as one of our ‘Teams of the Year’

The Kingston Satellite Dialysis and Transitional Care Unit has been awarded one of two Team of the Year honours at the Nursing and Midwifery Awards and Scholarships (NAMAS) 2025.
Their implementation of on-site dialysis for rehabilitation patients has been transformative. For example, by providing on-site haemodialysis for Kingston rehabilitation patients, the team has eliminated the need for 350 annual trips to Clayton, substantially improving rehabilitation outcomes.
Jane Crossett, Nurse Manager for Dialysis and Satellite Services, highlighted the team’s exceptional flexibility and courage throughout their service expansion journey.
‘Our team’s adaptability and unwavering commitment have been instrumental in creating a high-performing culture that directly benefits our patients,’ Jane stated.
The Kingston Satellite Dialysis and Transitional Care Unit has excelled in fostering multidisciplinary collaboration, integrating dialysis with rehabilitation care, and supporting new patients.
‘The new Model of Care, called the Transitional Care Unit, was planned over several years by our incredible Nephrology Leadership Team, who saw the vision and helped birth this programme at Kingston Dialysis,’ Jane said.
‘I’d like to extend my thanks to our former DCO, Jenny Soding, our current DCO, Leanne Linehan, the Unit Head, Professor John Kanellis, and our Acute Dialysis Nurse Manager from Clayton, Bronwyn Davis. We could not have done this without their support!’
Combined, their efforts have significantly reduced patient movement, improved rehabilitation outcomes, and elevated the overall care experience.
‘By embracing service needs without boundaries, we’ve cultivated trust, engagement, and an uplifting team culture. This teamwork ensures each patient feels respected and empowered throughout their care journey,’ Jane explained.
