Adult Diabetes outpatient services

We provide face to face and telehealth appointments. Your referral has been triaged by a senior endocrinologist to find you the most suitable clinic. We hold clinics in numerous sites to cater to our patient needs and we have a number of highly specialised clinics including Young adult clinic, Diabetes Kidney clinic and Diabetes in Pregnancy clinics.

We provide multidisciplinary care with our team of endocrinologists, DNEs and dietitian.

We will provide a comprehensive management plan that we will be shared with your GP. Our aim is to partner with you to assist you in achieving your health goals that can be continued and maintained with your GP.

Telehealth appointments

Please log onto telehealth appointments 10 minutes prior to your scheduled time to ensure you can navigate any technical issues.

Face to face appointments

Please aim to come to your face to face appointments 10-15 minutes prior to assist with checking in. Please do allow up to 2 hours to be seen, as we try to ensure that all patients have the opportunity to discuss all their concerns and diabetes is a chronic disease.

What to bring

Please ensure you bring the following:

  • Medicare card
  • Glucometer and blood glucose records
  • Up to date list of current medications
  • Any recent blood test reports that you may have
  • Any recent eye reports that you may have
