The Monash at Home – Aged and Rehabilitation Care service is expanding, and we’re looking for healthcare professionals from a range of backgrounds to join our team.
This service provides safe, high-quality subacute at-home care and multidisciplinary assessments for individuals facing complex medical, functional, and often cognitive challenges.
Acting as an alternative to hospital-based subacute care, Monash at Home enables patients to be discharged earlier from acute care or avoid a subacute hospital stay altogether.
Patients under this program receive daily visits from a member of the multidisciplinary team and have access to 24-hour telephone support.
The care model strives to provide short-term, high-quality, safe, and effective patient-centred care to individuals within the primary catchment area of Monash Health.
Excitingly, the program is currently undergoing a growth phase that involves expanding the service’s clinical capacity and extending its geographical reach.
