Post Acute Care (PAC) provides community-based services /home-based treatment and support to help patients recover at home after leaving hospital. It aims to assist people especially if they live alone, have limited support or if they are usually a carer. PAC services are available for up to 28 days post hospital stay.

- Patients are referred to the program during their hospital stay.
- Referrals are accepted up to 28 days after discharge.
