Post Acute Care (PAC) provides community-based services /home-based treatment and support to help patients recover at home after leaving hospital. It aims to assist people especially if they live alone, have limited support or if they are usually a carer. Where another program was providing support prior to the hospital episode, it is expected that these services will resume on discharge from hospital.

Services include:
  • Community nursing
  • Personal care
  • Home care
  • Physiotherapy

Eligibility Criteria

  • Patients are referred to the program during their hospital stay.
  • Referrals are accepted up to 28 days after discharge.
