Ophthalmology

• Address the referral to the Head of Unit: Dr Christine Chen
• The referral MUST be accompanied by visual acuity where possible and meet the Monash Health Ophthalmology Department tertiary referral guidelines to be accepted for triaging
• The referral should include a valid provider number and indicate that it is valid for an indefinite period
• The patient will be seen by a clinician within this specialty.
