IBD – Transition / Young Adult Service

The transition clinic (young adult IBD service) is a unique service in Victoria that looks after adolescents and young people with IBD, aged 16-25. Many of our patients have transitioned from Monash or the Royal Children’s Hospital IBD clinic. However, we also see many other patients from different pathways, including many young people first diagnosed with IBD aged 16-19.

The clinic is medically staffed by Dr Ed Giles, head of paediatric IBD at Monash Children’s Hospital, and Dr Lani Prideaux, consultant adult gastroenterologist, with the multi-disciplinary support of the Monash IBD service.

Why a dedicated young adult service?

Multiple studies and guidelines have stressed the importance of a dedicated transition service for IBD and other chronic medical conditions. Our service has great experience in this complex situation, supporting young people as they navigate education and employment issues while dealing with their IBD. As part of the Monash IBD service, we also offer opportunities patients to be involved in clinical trials and other IBD-related research.

We welcome referrals from any providers for confirmed or suspected IBD patients aged 16-19 years old.
