Complex Psychosocial Conditions (Care in Context)

Care in Context works with clients who are frequent presenters or at risk of presentation to Southern Health Emergency Departments who also have chronic disease and/or complex psychosocial issues. This is an outreach program which employs a multi-disciplinary team approach. The service is provided to both adults and children.

Care in Context provides:

  • A comprehensive and holistic approach to address health and other life issues
  • Links people into appropriate services and supports
  • Coordinates communication and care of client with health and service providers
  • Facilitates greater cohesion between acute, sub-acute and continuing care services i.e. GPs

Care in Context aims to:

Decrease inappropriate presentations to emergency departments and reduce length of stay when an inpatient.

A team also provides services to areas not covered by above Community Health Services. Acute Coordinators are based at Monash Clayton, Dandenong and Casey Hospitals and provide intake into the program.

Eligibility criteria

Adults and Paediatrics with three Emergency Department presentations in the last 6 months or at risk of presentation to an Emergency Department.
