Featured roles: Monash at Home, Aged and Rehabilitation Care

The Monash at Home – Aged and Rehabilitation Care service is expanding, and we’re looking for healthcare professionals from a range of backgrounds to join our team.

This service provides safe, high-quality subacute at-home care and multidisciplinary assessments for individuals facing complex medical, functional, and often cognitive challenges.

Acting as an alternative to hospital-based subacute care, Monash at Home enables patients to be discharged earlier from acute care or avoid a subacute hospital stay altogether.

Patients under this program receive daily visits from a member of the multidisciplinary team and have access to 24-hour telephone support.

The care model strives to provide short-term, high-quality, safe, and effective patient-centred care to individuals within the primary catchment area of Monash Health.

Excitingly, the program is currently undergoing a growth phase that involves expanding the service’s clinical capacity and extending its geographical reach.

Take a look below at the roles within the program we are currently hiring for, these positions are located in the southeastern suburbs of Melbourne, Victoria, and serve various Monash Health sites.

Right now, Monash Health is advertising for over 300 roles across all areas of healthcare. If you’re ready for a career that goes beyond the ordinary, and where you can make a real impact on the lives of others, take a look at all of our current vacancies or learn more at our Careers website.
