How we can help

We want to work with health professionals to facilitate the timely activation of high-quality supports for your patients and clients.

Monash Health has registered as an NDIS provider because we recognise the benefit of integrated health and disability support for NDIS participants. This is something that we, as Victoria’s largest health service, are uniquely able to offer to participants from birth to 65 years of age.

We are NDIS providers for

  • Support Coordination – Level 2 and Level 3 (Specialist Support Coordination)
  • Child and Family – Early Childhood Intervention and Children over 7 years including physiotherapy, occupational therapy, speech pathology, dietetics, podiatry, exercise physiology
  • Adults service including occupational therapy, dietetics, and speech pathology (click here for more about services offered)

Referral Criteria

  • All referrals need to have allied health hours specified in the NDIS participant’s current NDIS plan.
  • Services are available if the participant lives within a 30 minute drive of:
    • Springvale (for Support Coordination and Adult services)
    • Cranbourne (for Adult services and Child and Family Services)
    • Pakenham (for Adult and Child and Family Services)
  • Clients/NDIS participants who live further than a 30 minute drive of the the sites listed above may be referred but can access centre-based services only.

Our team

Our NDIS Support Coordination team are highly experienced experts in health and disability care, and have sound clinical knowledge and skills in allied health and case management.

The team have years of experience transitioning people to the NDIS They also have a thorough understanding and extensive knowledge of NDIS legislation, operational guidelines and principles.

Our team operate professionally and with integrity to ensure our NDIS participants are given the best options, and are empowered to choose the supports that will suit their needs.
