Services include an after-school ‘Youth Health Hub’ program at the Monash Health Community site in Dandenong. The program is held on Tuesdays and Thursdays. It aims to support and empower vulnerable young people aged around 15 -25 years to improve their health by providing individual support, youth-friendly health education groups and facilitating access to other community and health services.
Eligibility Criteria
- This service is for:
- People aged 15-25 years
- People who hold a healthcare or pensioner concession card, or who are a dependent of a concession card holder
- People with a low or medium income
- Aboriginal and Torres Strait Islander people
- Refugees and people seeking asylum
- People who are homeless or at risk of homelessness
- Children in care, child protection, Orange Door and ChildFIRST clients
