This program supports an individual’s ability to remain independent and living in the community by providing enjoyable and meaningful activities to support social inclusion and community participation.
The program helps to:
- keep the client independent within their community
- promotes health, independence, and self-management
- assist with timely interventions to prevent deterioration of their health conditions, and admission to hospital and or residential care. The interventions are client and family centred, and reflect culturally sensitive practice.
- Groups are available for men, women, mixed and those with mild-moderate Dementia.
- Groups are delivered in the centre and in the community with a mix of centre-based activities and outings.
Eligibility Criteria
This service is for people aged 18-65 with a disability or long-term impairment living in the City of Greater Dandenong, City of Casey or Cardinia Shire.
This service is not:
- available for Enhanced Primary Care (EPC) or Chronic Disease Management Plans
