Post Acute Care (PAC) provides community-based services /home-based treatment and support to help patients recover at home after leaving hospital. It aims to assist people especially if they live alone, have limited support or if they are usually a carer. PAC services are available for up to 28 days post-hospital stay.

Eligibility Criteria
This service is for:
- People over 18 years old who live or work within the Monash Health catchment and is willing to travel to centre-based services using their own means
- Clients who are medically stable and able to participate in a rehabilitation program
- Clients who consent to participate
- Clients who have been diagnosed with a mild, moderate or severe chronic lung disease
- Clients who have recently been discharged from hospital and are receiving treatment for the same condition
- Clients who have active and realistic rehabilitation goals
This service is not a primary referral for mental health, alcohol or drug dependency.
