Diabetes Education – Adult Community

Diabetes Nurse Educators and Dieticians provide:

• diabetes self-management support
• nutrition and dietary support
• understanding of medications and insulin therapy
• exercise and managing hypoglycaemia

Services are centre-based.

Eligibility Criteria

Adults over 25 years old who:

• attend a GP
• live or study living within the City of Greater Dandenong, City of Casey and Cardinia Shire.
