The Community Diabetes Nurse Educators (DNE) and Dieticians work as part of a multidisciplinary team.
They will work with clients to:Â
- teach them about diabetes self-management Â
- help them understand lifestyle changes to nutrition and exercise
- understand how to use medications, including insulin therapyÂ
- monitor and manage blood glucose levels
- identify other health problems they may be at risk of explain how these problems can be prevented
- coordinate their diabetes care with their general practitioner (GP) and other health professionals
Services are centre-based and short-term, and support people with diabetes in living well in the community through self-management of their conditions.
Eligibility Criteria
Adults over 25 years old who:
- have a confirmed diagnosis of diabetes
- attend a GP
- live or study living within the City of Greater Dandenong, City of Casey and Cardinia Shire.
