The Community Diabetes Nurse Educators (DNE) and Dieticians work as part of a multidisciplinary team.

They will work with clients to: 

  • teach them about diabetes self-management  
  • help them understand lifestyle changes to nutrition and exercise
  • understand how to use medications, including insulin therapy 
  • monitor and manage blood glucose levels
  • identify other health problems they may be at risk of explain how these problems can be prevented
  • coordinate their diabetes care with their general practitioner (GP) and other health professionals

Services are centre-based and short-term, and support people with diabetes in living well in the community through self-management of their conditions.

Eligibility Criteria

Adults over 25 years old who:

  • have a confirmed diagnosis of diabetes
  • attend a GP
  • live or study living within the City of Greater Dandenong, City of Casey and Cardinia Shire.
