Community Rehabilitation offers short-term goal-directed rehabilitation aimed at improving a client’s function and independence following a change in function due to recent acute medical / health event.
Streams include:
- General Rehabilitation
- Orthopedic Rehabilitation
- Neurological Rehabilitation
- Pulmonary Rehabilitation
- Oncology Rehabilitation
- Chronic Heart Failure Rehabilitation
- Cardiac Rehabilitation
Eligibility Criteria
This service is for people who:
- are 18 years and older.
- live in Cities of Greater Dandenong, Casey, Cardinia Shire and parts of the Cities of Kingston, Bayside and Monash.
- are medically stable and able to participate in a rehabilitation program
- find their overall ability to manage daily life has been affected by a recent health event that resulted in significant changes
- have functional rehabilitation goals.
- consent to participate.
Please Note: Where a client is being discharged from a hospital, the referrer is responsible for planning and implementing a safe discharge for the client including an Occupational Therapist home assessment and provision of equipment where necessary.
This service is not for:
- Primary rehabilitation for mental health, alcohol or drug dependency​
- People with acute mental health issues or severe cognitive impairment
