Community Health Nursing (18 to 65 years)

Introduction

Community Health Nursing is part of an interdisciplinary team that supports frail older people, younger people with disabilities and their carers. Care Coordinators (ICHT) assist complex clients to manage ongoing illnesses with coaching and support to follow through on healthcare recommendations.

The Living Well team based at Springvale have a particular focus on clients who are homeless or at risk of homelessness.

Eligibility Criteria

This service is for people aged 18-65 with a disability or long-term impairment living in the City of Greater Dandenong , City of Casey or Shire of Cardinia.

This service is not for Enhanced Primary Care (EPC) or Chronic Disease Management Plans

CHSP funded Community Nursing (65+ or 50+ Aboriginal and Torres Strait Islander)

Introduction

CHSP funded Community Nursing is available for people who are 65 and over or 50 years and over for Aboriginal and Torres Strait Islander people.

Community Nurses provide assessment and short-term care coordination for clients with complex care needs to assist them in navigating health and government services. If the person has multiple chronic health conditions which are impacting their ability to manage at home Community Health Nursing is often the best discipline to see the client first.

  • Services provided include:
    • Support and education for clients with poor health literacy who are not able to manage their health needs effectively
    • Health education and advice to caregivers regarding complex health conditions
    • Advocacy for clients who have had difficulty accessing medical or health care
    • Assessment of a client’s health needs
    • Medication Review and Management
    • Continence Support

Eligibility Criteria

This service is for:

  • people aged 65+ or
  • 50+ for Aboriginal and Torres Strait Islander people or
  • 50+ prematurely aged on a low income and homeless or at risk of homelessness or
  • 45+ Aboriginal and Torres Strait Islander prematurely aged on a low income and homeless or at risk of homelessness.

AND

  • who have difficulties performing activities of daily living without help due to functional limitations (e.g. social interaction, mobility,self-care).

This service is not for:

  • Enhanced Primary Care (EPC) or Chronic Disease Management Plans
