Services we provide include the Support at Home Progam and Commonwealth Home Support Program.

Our team understands that staying at home is important to you, which is why we take the time to get to know you, your preferences and partner with you to understand the care you require.

Whether you need basic home care or more complex healthcare support, we have a range of services and people to work with you. We will tailor a care plan around your approved support plan and circumstances, your needs and your interests.

Support at Home Program

The Support at Home program delivers government-funded aged care services designed to help older people live independently at home for longer. It provides coordinated, person-centred support for people with age-related needs, including those with more complex care requirements.

At Monash Health, we know that Support at Home looks different for everyone. That’s why we take the time to talk with you about your needs and what matters most to you.

You may need clinical supports such as nursing or allied health, or help preparing meals or transport to medical appointments. Whatever your situation, we’re here to make things easier.

When you contact us, your Personal Care Partner will guide you through the aged care system. We will work with you to understand your goals, explain your options, and create a flexible care and support plan that meets your needs and preferences. All our Support at Home participants have access to care management so that you to get the best outcomes from your aged care services.

Program Goals

Support at Home aims to help older people remain living safely and comfortably at home by:

  • improving access to services, products, equipment, and home modifications
  • advocating for higher levels of care for people with complex needs
  • ensuring person-centred care; recognising your needs, goals and care preferences
  • providing services such as:
    • Clinical supports, such as nursing care, physiotherapy, occupational therapy, and podiatry
    • Independence supports, such as help with showering, transport, and respite care
    • Everyday Living supports, such as cleaning, gardening, shopping, or meal preparation.

Eligibility

Support at Home is available to eligible people aged 65 years and over who need coordinated services to meet their assessed ageing-related care needs, or people aged 50 years or older who are:

  • Aboriginal and Torres Strait Islander
  • Experiencing homelessness or at risk of homelessness

You must have an Aged Care Assessment to access Support at Home services.

What the Support at Home Program can offer

The Aged Care Assessment will determine:

  • Your ageing-related needs and goals
  • Eligibility for ongoing services and/or short-term supports
  • Your funding classification and approved services
  • The priority category for your funding allocation

The Support at Home program provides:

  • Ongoing Home Support funding package: There are 8 levels, depending on your Aged Care Assessment (Level 1 to 8)
  • Three short-term care pathways:
    • Restorative Care Program
    • Assistive Technology and Home Modifications
    • End of Life Program

-The Restorative Care Pathway

The Restorative Care Pathway:

  • is a short-term, goal-oriented program designed to help older people recover or maintain their independence after an illness, injury, or change in health.
  • provides up to 16 weeks of coordinated nursing and allied-health services
  • supports you with your own “Restorative Care Partner” who works with you to set realistic goals and coordinate your care.
  • aims to reduce the need for long-term care by restoring your ability to perform daily activities and live more independently.
  • has a budget of around $6,000 (with potential to go up to $12,000).

- Assistive Technology and Home Modifications Short-Term Pathway

The Assistive Technology and Home Modifications Short-Term Pathway:

  • provides separate, government-funded support for equipment and home changes that help you live safely and independently.
  • is available for a defined period (usually 12 months).
  • includes assistive technology like mobility aids (e.g. walking frames or wheelchairs) and toileting supports. Maximum $15,000
  • includes home modifications such as bathroom modifications or grab-rails, ramps, or even stair lifts.

While you do not have to pay for allied health assessments and prescriptions, you may have to contribute to the cost of equipment and home modifications depending on your income level.

- End of Life Pathway

The End-of-Life Pathway under the Support at Home program helps older people to remain at home in their last 3 months of life. It can be accessed through a referral from a doctor or nurse practitioner. This pathway provides:

  • additional short-term (12 weeks) support for older people who have been medically assessed as having limited time to live and wish to remain at home.
  • services such as personal care, domestic assistance, nursing, and assistive technology.
  • a care partner who will work closely with you, your family, and palliative care teams to create a responsive support plan that helps you remain comfortable at home.

 

Information for Transitioning Home Care Package clients

The Support at Home program has replaced the Home Care Packages (HCP) program.

  • If you previously received a Home Care Package, you transition automatically into the new Support at Home Program.
  • You do not have to get a new Aged Care assessment unless your needs change.
  • Your Support at Home budget will match the annual package amount you get now, but the new budget will be split into four equal quarterly budgets.
  • Your Personal Care Partner will support you through this transition by:
    • helping you understand any changes and
    • making sure your services continue smoothly.

Commonwealth Home Support Program (CHSP)

The Commonwealth Home Support Program (CHSP) can support you at home, based on your needs and preferences. The CHSP provides entry-level support to help older people continue to live safely and independently at home and in their communities. We will talk with you to understand your needs, discuss your options and create a care plan that works now and can be reviewed in the future as your needs and preferences change.

Our experienced team of health professionals provides care for people aged 65 and over and for Aboriginal and Torres Strait Islander people aged 50 years and over, who have a range of healthcare needs.

CHSP is not designed for people with intensive or complex care needs. People with higher needs are supported through other aged care programs such as the Support at Home Program (which was known as Home Care Packages before 1 November 2025) and residential aged care.

If you are caring for an older person who is frail, living with dementia, has a disability or ill-health, we are here to help with a range of services to support carers and their loved ones. We can guide you through service options, obtain referrals to health professionals and connect you with our coordinated health or residential care services.

Our allied health programs offer occupational therapy, physiotherapy, exercise physiology, podiatry, speech pathology, social work and dietetics to support you to remain independent and well at home.

Our community nursing program offers:

  • Short-term wound management
  • Health assessments
  • General continence product advice and funding applications
  • Medication reviews
  • Personal alarm assessments
  • Carer support.

We also offer several centre-based, home-based and group programs to support your needs including:

    • Falls prevention (exercise and education)
    • Exercise based programs (such as osteoarthritis, strengthening and reconditioning, neurological)
    • Communication/Aphasia

  • The pathway for accessing all government funded aged care services is through My Aged Care. You can contact My Aged Care via the website or by calling 1800 200 422 (Monday to Friday 8am to 8pm; Saturday 10am to 2pm).
