We ask community health practitioners to note that Monash Health Imaging will soon be renamed as Monash Health Radiology. You can continue to use existing Monash Health Imaging request forms and we will continue to accept complete referrals on any provider request form.
We’re excited to share that Monash Health Imaging will soon be known as Monash Health Radiology. This change will be introduced gradually across our online platforms and printed materials as current referral stocks are used.
In the meantime, please continue to use your existing Monash Health Imaging request forms. We will also continue to accept complete referrals on any provider request form.
Our commitment to patients
We remain dedicated to supporting as many patients as possible and prioritising imaging based on clinical urgency. To help us manage the high volume of referrals from both Monash Health and the community, we kindly ask that all imaging requests be completed in full.  It is of utmost importance to include a clinical question, the clinical indications and notes.

Referral checklist
Please see the checklist below of key information to include on referrals, to help reduce the risk of referral rejection. Including additional important information for contrast scans will also support timely processing and the safest possible care:
- For scans requiring contrast, please include:
- the patient’s eGFR
- history of overactive thyroid
- current medications
- recent thyroid function results.
- For patients with a contrast allergy, please include the symptoms experienced during the reaction.
- Previous imaging and provider details.
