Monash Health is partnering with Deakin University to improve care for patients with heart failure (HF) through the SAFE‑HF trial (tranSlating heArt Failure guidElines into practice), a randomised controlled Nurse Practitioner service.

The service is available to General Practitioners (GP) within the Casey and Cardinia region and aims to strengthen translation of HF guidelines into practice through implementing and testing a new system of care.

Eligible patients hospitalised with heart failure, or referred by their GP, receive early structured follow‑up from a Heart Failure Nurse Practitioner, commencing one week after hospital discharge or at referral, and continuing weekly for up to twelve weeks.

Care focuses on medication optimisation, symptom management, patient self‑care education, and development of individualised management plans.

SAFE‑HF is designed to support GPs in managing patients with heart failure during the high‑risk post‑discharge period, while strengthening shared care between primary care and specialist services.
