Interventional care for adults with chronic illness

Summary

We welcome GPs to refer patients with chronic illness to the Monash Health's Community Assessment Response Team (CART) Primary Care and Support program, which provides interventional care for adults aged 18 to 65 suffering from chronic illness.

Monash Health’s Community Assessment Response Team (CART) Primary Care and Support program provides interventional care for adults aged 18 to 65 suffering from chronic illness.

Care Coordinators collaborate with GPs and other healthcare professionals to ensure patients are receiving holistic health management, and provide:

  • comprehensive assessments
  • care planning
  • health promotion and education
  • service coordination and referral
  • advocacy and support
  • support with the utilisation of self-management principles.

Patients who would benefit from care coordination support include those:

  • not eligible for NDIS
  • with complex health conditions
  • with poor health outcomes.

There is no cost for this service.
