Transition care program

The Transition Care Program is a short term program providing care and assistance with recovery for older people who have been in hospital.

By offering low level therapy and support, the program allows people to be discharged quicker and continue their recovery out of the hospital system, while long term care is arranged.

Services can be provided in your own home or a nursing home facility. The time on the program is dependent on individual care goals, however the limit is twelve weeks.

Eligibility

To access the program, people must:

  • be medically stable and have completed their acute and/or sub-acute hospital stay
  • be able to make some functional gains or benefit from a period of active management to complete the restorative process
  • have been assessed by the Aged Care Assessment Service (ACAS)
  • wish to enter the program
Please note that from 27 August 2024 until late October, Clayton Road is completely closed to all non-emergency traffic between Monash Medical Centre and Haughton Road, just south of the railway line.Learn more
