The Aged and Community Care Program is a primary community-based resource which provides an interdisciplinary service to support frail older people, younger people with disabilities and their carers.
The program aims to keep the client independent within their community, proactively promoting health and independence, and self-management whilst assisting with timely interventions to prevent deterioration of the condition, and admission to hospital and or residential care.
The purpose of the Aged and Community Care Program is to assist people whose lives are impacted by ill-health or frailty, and those who care for them, to live fulfilling and healthy lives safely in their own home environment. An initial broader needs assessment will be undertaken and a plan made with the client outlining the services to be provided.
Individual and group based programs are available and a range of different clinical services are available. Treatment may be at either of our centres or may occur in the client’s own home or another community venue.

$10 per session for pensioners and health care card holders
The program is HACC funded. It services people over 65 years living in the City of Greater Dandenong.
