Monash Health has an array of dedicated services for Aboriginal and Torres Strait Islander patients during their hospital stay. It is important that you let your medical team know if you or your family identify as Aboriginal and/or Torres Strait Islander to ensure the Aboriginal Health Team can support you during your medical treatment.

Aboriginal Hospital Liaison Officers

The role of an Aboriginal Hospital Liaison Officer is to provide cultural support and assistance to all Aboriginal and Torres Strait Islander patients and their families during their inpatient stay in Monash Health. They can also assist in connecting patients to community services during discharge planning.

We offer:

  • Cultural, emotional and spiritual support to patients and families
  • Participation in family meetings, goals of care and care planning
  • Patient advocacy and referrals to other services
  • Support in relation to discharge planning and linking to community services.

Eligibility Criteria

Patients or families of Aboriginal and Torres Strait Islander background. There are no catchment or boundary restrictions.

Access and Support Workers

The Access and Support workers offer support for Aboriginal people support to accessing support to maintain their health, wellbeing and independence, including navigating the My Aged Care and NDIS service system, in addition to transportation to appointments and accessing community based services.

Dietetics

Eligibility Criteria

Patients or families of Aboriginal and Torres Strait Islander background. There are no catchment or boundary restrictions.

The Aboriginal Health Dietician provides Aboriginal community advice and support to make improvements to their health through nutrition. This may be done in a variety of ways and could include individual consultations and/or community education.

Healthy Koori Kids

Healthy Koori Kids Healthy Koori Kids service for Aboriginal children aged 0-18 years who are at risk of entering, or are in out-of-home care. The service offers a multidisciplinary team of skilled professionals consisting of a paediatrician, paediatric psychologist, speech therapist, oral hygienist and Aboriginal Health Workers, providing a fortnightly clinic to address children’s health needs.

Eligibility Criteria

Aboriginal and/or Torres Strait children in out of home care, or at risk of living in the southern metropolitan region. There are no catchment or boundary restrictions.

Aboriginal Midwife Coordinator

The role of the Aboriginal Midwife Coordinator is to provide culturally safe maternity care to Aboriginal women, babies and families during the course of pregnancy as well as post-natal care.

Support offered:

  • Cultural, emotional and spiritual support to women and families.
  • Participation in pre-natal and post-natal care planning and appointments
  • Culturally safe maternal and parental education and clinical support
  • Patient advocacy and referrals to other services
  • Support in relation to appointments, birth plan and maternal and child health follow up.

Eligibility Criteria

Aboriginal and/or Torres Strait Islander women or women pregnant with Aboriginal and/or Torres Strait Islander babies. The woman can be referred at any stage of pregnancy. There are no catchment or boundary restrictions.

Aboriginal Paediatric Nurse

The role of the Aboriginal Paediatric Nurse is to provide support to Aboriginal children, parents and families during their inpatient stay as well as outpatient appointments and in community.

Support offered:

  • Cultural, emotional and spiritual support to children, parents and families.
  • Culturally safe clinical support during family meetings, appointments and ward rounds
  • Patient advocacy and referrals to other services
  • Support in relation to discharge planning and services in community.
