The Community Program connects acute hospital care and community-based care. Through our services we support people’s wellbeing and opportunities for them to live their best lives.
We help people stay healthy and well by being part of the communities where people live, work and play. We match medical and other needs with targeted, multidisciplinary care so we can deliver a connected system of care around their needs and where they live. This is supported by services that enable people to care for themselves and make healthy choices.
Our services are offered face-to-face and in group settings and include clinical (eg: nursing, dental, medicine, physiotherapy, occupational and other therapies) and non-clinical (eg: counselling, community development, social work, case management) services. Some services are also available through telehealth.
