Adult program

Monash Health has been providing health care for more than 100 years and is the largest healthcare service in Victoria.

With more than 30 years’ experience working with people with disabilities, Monash Health NDIS participants benefit from access to our highly skilled staff and continuous support across both disability and health care services.

We know how much you value living a fulfilling life in your own community, and our goal is to build your capacity to do this.

Our services

Monash Health can provide the following services for adults.

Other Services

Capacity Building

Capacity Building Supports by a Therapist. This helps you towards increased functional independence and social participation. Therapies include: Physiotherapy, Occupational Therapy, Dietetics, Speech Pathology, Podiatry and Exercise Physiology.

Non Face-to-Face activities

Non face-to-face activities that are part of delivering NDIS Support Coordination. Examples of this include:

  • research into NDIS community services
  • sourcing NDIS community services
  • communication about NDIS supports and services such as telephone calls, emails and SMS
  • training on how to use the NDIS participant portal and understanding a NDIS plan

Report Writing

  • Writing National Disability Insurance
  • Agency (NDIA) required reports


Travel to deliver supports to a participant:

  • in their home and/or community
  • up to a maximum of 30 minutes per trip.

Depending on what you need, Monash Health can see you in your home or at one of our sites:

  • Springvale – 155 Buckingham Avenue.
  • Cranbourne – 140-154 Sladen Street.
