Internal Referrals Only: Children must already be referred to, or actively engaged with, another service within the Child and Family Program to be eligible for referral.
Social Workers support children aged 0–17 years and their families by providing psychosocial support and advocacy. This is delivered through comprehensive assessment, counselling, information provision, and facilitation of access to community resources. Social Workers are integral members of the multidisciplinary team and play a pivotal role in cases involving complex social circumstances.
- Advocacy and support to navigate access to internal and external community services and providers
- Assistance with social issues and referral to specialist supports
- Parenting support and information
- Support for families adjusting to major life changes and significant stressors
- Linking families with services such as material aid
- Support with routines and behaviour management
Eligibility Criteria
This service is for:
- children aged 0-17 who live in the City of Greater Dandenong, City of Casey, or Cardinia Shire
- children who are currently engaged in support with another service(s) within the Child and Family Program and/or
- children engaged with Refugee Health and/or Monash Health community paediatrics
This service is not for:
- children who come from high-income families
- children who are not part of another Child and Family Program at Monash Health
- children currently linked with Early Childhood Early Interventions (ECEI) or National Disability Insurance Scheme (NDIS)
- children who do not live, work or attend kinder or school in the Monash Health catchment.
