Adult Occupational Therapy assists people and carers whose lives are impacted by ill-health or frailty, to live fulfilling and healthy lives safely in their own home environment.

Community-based Occupational Therapy offers:

Assessments and interventions to improve function and safety with activities of daily living in the home environment and community
Home environment optimisation (e.g., home modifications, aids, and equipment)
Falls prevention assessment and education
Education to improve engagement in daily living activities

All of our services are provided as part of a comprehensive integrated Allied Health Team which can refer clients to other services and group activities to help improve health, wellbeing and social connection.

Eligibility Criteria

This service is for:

people aged 18-65 with a disability or long-term impairment living in the City of Greater Dandenong, City of Casey or Cardinia Shire.

Referrals are not suitable if:

Condition Onset:

Sudden/acute onset of deterioration, including increased pain or loss of function, is reported

Referral Type:

Referral is for hand therapy, return to work program, vehicle modifications, or driving assessment.
Referral is primarily for a report related to NDIS/DSP access.
Referral is primarily for formal assessment of cognition.
Referral is for mental health conditions or intellectual disability as the only primary presenting condition.
Referral is primarily for mobility aids (e.g., gait aid).

Referral Details:

No clear OT goals are indicated in the referral.
Individual is receiving NDIS, TAC, or palliative services for the primary condition.
Individual is already receiving OT input from another public, private, or community health service for the same needs.
Referral is for a home maintenance issue (e.g., cracked path, worn carpet) that falls under the responsibility of the homeowner or landlord.

Suggested referral pathway:

Suggest urgent medical attention via Emergency Department or Urgent Care Centre.

Suggest GP appointment for redirection to an eligible service

Referral to CART Occupational Therapy service is not appropriate.
