Clients must live in the Cities of Greater Dandenong, Casey, Bayside or Kingston, or Cardinia Shire.
There is no minimum age for accessing a Home Care Package, however, a free assessment by an Aged Care Assessment Service (ACAS) is required. For more information about assessments please click here or call 1800 200 422.
In general, a person may qualify for a Home Care Package if they:
  • are an older person who needs some help to stay in their own home
  • are struggling with some aspects of living at home, but still able to manage most of the time
  • might have to go into an aged care home before they are ready because they do not have help at home
In exceptional circumstances, a younger person with a disability, dementia or other special care needs that are not met through specialist services may, with approval from the Department of Health Human Services (Victoria), apply for an ACAS assessment.

How much does a Home Care Package cost?

The Australian Government pays for most aged care services in Australia. From 1 July 2014, those who can afford to may need to make a contribution towards the cost of their care. As each client’s circumstances are different, the individual costs will also be different. Please call 9792 7747 or email communitysupportoptions@monashhealth.orgfor a personalised explanation of costs.
