Monash Health GP Liaison is dedicated to ensure correspondence is sent to General Practice. If you have voluntarily registered your GP details and clinic on to the National Health Services Directory, your registration will be added to the Monash Health database and updated accordingly.

28 June – South Eastern Cardiac Services Partnership Survey

The South Eastern Cardiac Services Partnership has been established to support General Practice to deliver Primary Care in the community, and manage Heart Failure and Arrhythmia.
The South Eastern Cardiac Services Partnership (including Monash Health, Alfred Health, Peninsula Health and all health services in Gippsland) was established to carry out the Cardiac Plan for South Eastern Victoria.
The Cardiac Plan was released in 2016, and provides a clear picture of the future of the statewide cardiac care system and ongoing reform to ensure people with heart disease achieve the best possible outcomes, in a health system that is efficient and of high quality.
By completing the following survey, you will help us understand and develop strategies to address:

  • Barriers to specialist care
  • Education and support requirements
  • Opportunities for improvement
