Hospital Outreach Medication Review (HOMR) conducts a home visit by a hospital pharmacist to comprehensively assess all aspects of a client’s medication management. This includes assessing the patient’s ability to manage their medications and medication aids. Education about medicines and health conditions is also provided. Recent inpatient admissions as well as outpatient appointments are reviewed to enable the HOMR pharmacist to explain medication changes, possible omissions and potential ways to optimise the patient’s medication regimen.
Phone consultation or referral for community Home Medicines Review is made where a home visit is not feasible due to distance or safety concerns. A report is sent to the patient’s General Practitioner and community pharmacy. Copies of the report are also sent to any specialist doctors or case managers involved in patient care.
